Conciliação medicamentosa na unidade de internação do Centro de transplante de medula óssea - CEMO/INCA

Abstract

Através da conciliação medicamentosa, o farmacêutico pode desempenhar um importante papel no gerenciamento da farmacoterapia dos pacientes para o alcance de bons resultados clínicos. Objetivo: Descrever os resultados da conciliação medicamentosa a pacientes admitidos em um Centro de Transplante de Medula Óssea. Método: Trata-se de um estudo transversal descritivo com abordagem quantitativa por meio de entrevistas a pacientes admitidos a fim de obter o histórico medicamentoso. As discrepâncias destes com as prescrições da admissão hospitalar foram identificadas como intencionais ou não intencionais, e classificadas segundo o tipo. As intervenções farmacêuticas realizadas junto ao prescritor foram categorizadas e os desfechos obtidos quantificados. Avaliou-se o impacto dos erros de medicação segundo critérios definidos para medicamentos de alto riscos por Penm et al. (2018). Resultados: Das 142 discrepâncias encontradas, 19% foram não intencionais. Destas, 81,5% foram por omissão do medicamento. Das intervenções farmacêuticas realizadas, 67,8% não foram aceitas pelo prescritor, todas de caráter intencional, sendo a principal justificativa conduta médica (34,4%). Das intervenções farmacêuticas aceitas, 51,7% eram para inclusão do medicamento omitido. Dentre os 29 erros prevenidos, 13,8% envolviam medicamentos classificados como de alto risco. Conclusão: Os resultados sugerem que o processo de conciliação medicamentosa pelo farmacêutico em pacientes submetidos ao transplante de células tronco hematopoéticas é uma importante ferramenta na prevenção de erros de medicação e otimização da farmacoterapia

Through medication reconciliation, the pharmacist can play an important role in the management of the pharmacotherapy of their patients to achieve good clinical results. Objective: Describe the results of the medication reconciliation to patients admitted to a Bone Marrow Transplant Center. Method: This is a cross-sectional descriptive study with a quantitative approach by means of interviews with patients admitted to obtain their medicinal history. The discrepancies of these with the prescriptions of admission have been identified as intentional or unintentional, and classified depending on the type. The pharmaceutical interventions performed with the prescribing were categorized and the obtained outcomes were quantified. The impact of the medication errors found were evaluated according to defined criteria for high-risk medications by Penm et al. (2018). Results: From the 142 discrepancies found, 19% were unintentional. From these, 81,5% were by omission of the medicinal product. Out of the pharmaceutical interventions performed, 67,8% were not accepted by the prescriber, all of them of an intentional nature, being the main justification medical conduct (34,4%). Out of the accepted pharmaceutical interventions, 51,7% were for inclusion of the omitted medicine. Of the 29 prevented errors, 13,8% involved medications classified as high risk. Conclusion: The results suggest that the medication reconciliation process by the pharmacist in patients submitted to stem cell transplantation is an important tool in the prevention of medication errors and optimization of pharmacotherapy.
