Correlação entre métodos de avaliação nutricional subjetivos e objetivos e o perfil imune de pacientes com câncer gástrico em pré operatório

Abstract

Objetivo: Correlacionar o estado nutricional, através de métodos objetivos e subjetivos, com o perfil imune de pacientes com tumores gástricos em pré-operatório. Metodologia: Estudo transversal, com dados de uma coorte com pacientes atendidos no ambulatório de Nutrição do HC I do INCA, em 2012 e 2013. Foram coletados os dados de peso e estatura para classificação do índice de massa corporal (IMC); dobra cutânea para avaliação da massa magra, pela circunferência muscular do braço (CMB), classificada como depleção moderada (p5-p10) ou grave (<p5); e ASG-PPP, que classifica os pacientes bem nutridos, risco ou moderadamente desnutridos e gravemente desnutridos. A resposta imune foi avaliada pelo hemograma, contagem linfocitária total, quantificação de células T, CD4, CD8 e células NK. Os dados foram analisados no programa SPSS, 17.0. Resultados: Avaliou-se 124 pacientes, em sua maioria, idosos (58,9%), do sexo masculino (65,3%), sendo 36,2% tabagistas e 41,7% etilistas. 55,6% tiveram ASG-PPP B e 14,5% ASG-PPP C. A maioria dos pacientes era eutrófico pelo IMC (46,8%), PCT (68,5%) e CMB (65,3%). A média da albumina foi de 4,12±0,67 g/dL, da pré-albumina 0,22±0,96 g/L, da proteína C-reativa 1,82±3,86 mg/dL, do CD4+ 38,12±9,09, do CD8+ 22,40±9,53 e da relação CD4/CD8 2,04±1,01. Diferenças estatisticamente significativas foram encontradas (entre bem nutridos e desnutridos), sendo menores para peso corporal, IMC, albumina, PCR e CD4. O escore da ASG-PPP apresentou fraca correlação com o CD4, enquanto os outros parâmetros apresentaram correlação muito baixa. Conclusão: O escore da ASG-PPP mostrou-se o melhor parâmetro correlacionado com o perfil imune de pacientes com tumores gástricos

Objective: Correlate the nutritional state, by objective and subjective methods, with the immune profile of patients with gastric tumor in the preoperative period. Methods: Cross sectional study, with data from a cohort study with patients treated in the outpatient clinic of Nutrition of HC I of the INCA, in 2012 and 2013. Data was collected in weight and stature for classification of body mass index (BMI); skinfolds for evaluation of lean mass, by arm muscle circumference (MAMC), classified as moderate depletion (P5-P10), or severe (<p5); and ASG-PPP, that classifies patients in well-nourished, risk or moderately malnourished severely malnourished. The immune response was evaluated by blood count, total lymphocyte count, quantification of T cells, CD4, CD8 and NK cells. The data were analyzed by SPSS program, 17.0. Results: We evaluated 124 patients, in their majority, elderly (58.9%), males (65.3%), being 36.2% were smokers and 41.7% etilists. 55.6% had PG-SGA B and 14.5% PG-SGA C. The majority of patients was eutrophic by BMI (46.8%), PCT (68.5%) and CMB (65.3%). The average of albumin was of 4.12±0.67 g/dL, pre-albumin 0.22±0.96 g/L, C reactive protein (1.82±3.86 mg/dL, the CD4+ 38,12±9,09, the CD8+ 22.40±9.53 and CD4/CD8 ratio 2.04±1.01. Statistically significant differences were found (among well nourished and malnourished), being lower for body weight, BMI, albumin, PCR and CD4. The score of PG-SGA presented poor correlation with CD4, while the other parameters showed very low correlation. Conclusion: The score of ASG-PPP showed to be the best parameter correlates with the immune profile of patients with gastric tumors.
