Retalho pediculado supraclavicular nas cirurgias reparadoras de cabeça e pescoço: Revisão bibliografica

Abstract

A reconstrução da cabeça e do pescoço é fundamental devido à sua importância durante o contato social e o fato dessas regiões serem áreas de movimentação. Estas devem respeitar as unidades estéticas e os retalhos devem ser finos a fim de permitir a mímica facial. Devemos priorizar o ângulo cervicomandibular por razões estéticas e para evitar deformidades maiores. Gillies, em 1920, enfatizou que quanto mais próxima for a área doadora, melhor será a mimetização da pele local original2 . A região do ombro e adjacências possui uma textura e coloração semelhantes à da face. O retalho supraclavicular é apresentado como uma versátil opção para as reconstruções cervicofaciais. O retalho fasciocutâneo supraclavicular foi abandonado por anos devido ao desconhecimento e imprevisibilidade de sua anatomia. Após Pallua et al publicarem estudo detalhado sobre a anatomia e segurança do retalho, no final dos anos 901 , as suas indicações se expandiram. Para defeitos superficiais, a região do ombro e adjacências oferecem uma segura opção de retalho fasciocutâneo. O objetivo deste trabalho é demonstrar a aplicabilidade do retalho supraclavicular nas diversas formas de reconstrução de cabeça e pescoço através de revisão bibliográfica. O retalho supraclavicular pediculado em ilha demonstra, segundo a literatura, ser versátil e de fácil execução, permitindo a sua ampla utilização em reconstruções de cabeça e pescoço com excelentes resultados estéticos e funcionais. A técnica cirúrgica é simples, dada à constante anatomia dos vasos envolvidos no retalho. Apresenta como vantagem sobre outras técnicas a possibilidade de ser confeccionado em centros de baixa complexidade a um custo relativamente baixo, de maneira rápida e previsível.

The reconstruction of the head and neck is fundamental due to its importance during social contact and the fact that these regions are areas of movement. These should respect the aesthetic units and the flaps should be fine in order to allow facial mimic. We should prioritize the cervicomandibular angle for aesthetic reasons and to avoid major deformities. Gillies, in 1920, emphasized that the closer the donor area, the better the mimic of the original local skin2. The region of the shoulder and adjacencies has a texture and coloration similar to that of the face. In this way, we present the supraclavicular flap as an interesting and versatile option for cervicofacial reconstructions. The supraclavicular fasciocutaneous flap was abandoned for years due to the unknown and unpredictable anatomy. However, after Pallua et al published a detailed study on the anatomy and safety of the flap in the late 1990's, their indications expanded. For superficial defects, the shoulder region and adjacencies offer a safe fasciocutaneous flap option. The objective of this work is to demonstrate the applicability of the supraclavicular flap in the various forms of head and neck reconstruction through a bibliographic review. According to the literature, the supraclavicular flap on the island is versatile and easy to perform, allowing its wide use in head and neck reconstructions with excellent aesthetic and functional results. The surgical technique is simple, given the constant anatomy of the vessels involved in the flap. It presents as advantage over other techniques the possibility of being made in centers of low complexity at a relatively low cost, in a fast and predictable way
